Every Indian citizen aged 70 or above can enrol regardless of income. Here is what the ₹5 lakh family-based cover pays for, how existing PM-JAY and government-scheme members are treated, and what to verify before hospital admission.

The Ayushman Vay Vandana Card extends Ayushman Bharat PM-JAY to Indian citizens aged 70 or above, irrespective of income. But “₹5 lakh free cover” is often misunderstood. It is not cash paid to the beneficiary and it is not a promise that every medical bill will be reimbursed. It provides cashless access, up to the applicable annual family limit, for listed secondary and tertiary hospital-care packages at empanelled facilities. Enrolment, hospital participation and package authorisation all matter.

The answer in 60 seconds

  • Eligibility: Indian citizens aged 70 or above, based on the age recorded in Aadhaar, regardless of economic status.
  • Document: NHA's FAQ says Aadhaar is the only enrolment document and Aadhaar-based e-KYC is mandatory.
  • Benefit: up to ₹5 lakh per year on a family basis for listed cashless hospital packages at empanelled public or private hospitals.
  • Existing PM-JAY families: members aged 70+ receive an additional top-up of up to ₹5 lakh reserved for them, separate from the regular family cover used by younger members.
  • Other government health schemes: people using CGHS, ECHS or Ayushman CAPF must choose their existing government scheme or AB PM-JAY. People with private insurance or ESIC can still use Vay Vandana.
  • Start date: NHA says treatment can be accessed from the first day after enrolment, with no waiting period.
  • Latest official scale: 1.20 crore senior citizens were enrolled and more than 13.84 lakh treatments worth ₹3,000 crore had been availed as of June 5, 2026.

Who qualifies?

The age threshold is 70 years or above. Eligibility does not depend on income, caste, rural residence or inclusion in the Socio-Economic Caste Census. The age shown in Aadhaar is used. NHA's FAQ adds a special rule where Aadhaar records only a birth year: January 1 of the year following that recorded year is treated as the date of birth for the eligibility calculation.

Both parents do not need entirely separate family enrolments. NHA says that after the first eligible 70+ family member is enrolled, another eligible senior can be added through the portal's “Add Member” function. Each eligible senior receives a distinct Ayushman card, while the financial benefit is described on a family basis.

What the ₹5 lakh covers—and what it does not

The benefit is cashless hospital care under approved PM-JAY packages. The national package master covers secondary and tertiary care across specialties, and pre-existing conditions are covered from day one. The hospital normally checks eligibility, selects the relevant package and seeks any required pre-authorisation.

The ₹5 lakh is not deposited into a bank account. It is not an unrestricted medical wallet, and routine outpatient consultations, medicines bought independently or treatment outside the package and empanelment system should not be assumed covered. Before a planned admission, ask the hospital's Ayushman help desk whether the facility is currently empanelled for the required specialty, whether the procedure maps to an active package and what authorisation is needed.

The top-up rule for families already under PM-JAY

A family already eligible under regular AB PM-JAY has its usual family cover. When members aged 70 or above enrol for Vay Vandana, the senior members receive an additional top-up of up to ₹5 lakh per year that they do not share with family members below 70. The official Cabinet note describes this as additional top-up cover for the eligible seniors; it should not be multiplied into ₹5 lakh for every individual senior unless the applicable scheme record expressly shows that.

What if the senior already has other health cover?

  • CGHS, ECHS or Ayushman CAPF: the senior must choose between the existing public scheme and AB PM-JAY.
  • Private health insurance: the senior remains eligible for Vay Vandana.
  • Employees' State Insurance: NHA's FAQ says an ESIC beneficiary is also eligible.

These rules describe scheme eligibility. They do not guarantee that the same bill can be claimed twice. Ask the hospital and insurer how coordination applies to a particular admission.

How to enrol safely

  1. Go directly to beneficiary.nha.gov.in or use the official Ayushman App published by the National Health Authority.
  2. Select the senior-citizen enrolment route and complete Aadhaar-based e-KYC.
  3. Add another eligible 70+ family member where applicable and download the distinct card after approval.
  4. Do not pay an “activation” or registration fee. Use official channels or a recognised assistance point.
  5. For help, call 14555 or give a missed call to 1800-110-770.

Before treatment: four checks that prevent surprises

  1. Find an empanelled hospital: use the official portal or app, not an old forwarded list.
  2. Confirm the specialty: a hospital may be empanelled without offering every PM-JAY specialty or package.
  3. Ask about authorisation: confirm that the planned procedure maps to a covered package and whether pre-authorisation has been obtained.
  4. Keep records: preserve the card, admission papers, authorisation details, discharge summary and grievance number.

Portability allows eligible beneficiaries to use empanelled hospitals outside their home state. The official hospital network changes, so a national total is not a substitute for checking the particular facility on the day care is needed.

Latest rollout numbers

A March 17, 2026 Health Ministry update recorded 1.14 crore Vay Vandana cards and 36,229 empanelled hospitals as of February 28. A later government overview said 1.20 crore senior citizens had enrolled and received more than 13.84 lakh treatments worth ₹3,000 crore as of June 5, 2026. These figures show substantial use, but they do not prove that every eligible senior has practical access nearby.

Claim versus evidence

  • True: income is not an eligibility test for citizens aged 70+.
  • True with qualification: cover is up to ₹5 lakh per year on a family basis through listed cashless packages—not ₹5 lakh cash for each person.
  • True: Aadhaar e-KYC is mandatory under NHA's published enrolment FAQ.
  • True: there is no treatment waiting period after enrolment, according to NHA.
  • Misleading: “any treatment at any hospital is free.” Treatment must fit the scheme's package and empanelment rules.

Frequently asked questions

Is 70 completed years required?

Yes. Eligibility begins at age 70, calculated from the age recorded in Aadhaar under NHA's rules.

There is no enrolment fee. Avoid unofficial links or agents demanding payment for activation.

Are pre-existing diseases covered?

PM-JAY says pre-existing conditions are covered from day one, subject to the relevant listed hospital package and authorisation.

Is outpatient care automatically covered?

No. PM-JAY is primarily structured around listed secondary and tertiary hospital-care packages. Do not assume every clinic visit, test or medicine purchase is reimbursable.

Where can a family complain?

Use the grievance feature in the Ayushman App or call the official helpline 14555. Keep hospital and authorisation records.
